  14. I agree that, in the event I do not pass the certification exam, the training center that provided the mandatory training cannot be held responsible, and I hereby relinquish any claims, indemnifications or return of the amounts invested.
  15. I am aware that to be approved in the certification exams I have to get 75% of right answers from the total exam questions, in the time stipulated by HUCMI. If the student reaches the time limit of the certification exam without the completion of 100% of the questions and the button to complete and submit the exam for verification is not pressed, he/she will be considered not approved and one intent of the certification test will be considered performed.
  16. I agree to carry out the certification exam remotely over the internet through the website of HUCMI (www.hucmi.com) at any location chosen by me. It is my fully responsibility the proper functioning of resources, such as computers, internet access or any other technological resources necessary to perform the examination.

  18. I agree that after the expiry of 180 days for the completion of the certification exam, and being within a period not exceeding one year from the date of completion of the training, I can request an additional period of 90 days for completion of the certification examination, upon payment of an administrative fee in the amount of US$ 50.00 (fifty dollars).

  21. I acknowledge that only two additional certification exams for any of the levels will be held if I do not pass the first one, no matter if it is in the period of 180 days after training or in an additional three-month period requested by me.
  22. I agree that to take another certification exam after three attempts in which I have not achieved the minimum score required by HUCMI, I will have to attend training again, restart the entire certification process and incur all related costs again.
  23. I acknowledge that, once I have selected a Training Center to take the training and pay for it, changing the Training Center will not be allowed.
  24. I understand and agree that once I have started the certification exam online (via the Internet) it cannot be restarted for any reason whatsoever and is considered one of the three attempts I have to carry exam of certification.
  25. I acknowledge that all the materials, tangible or digital, I send to HUCMI become a property of HUCMI and can be published on the HUCMI website or any other media without creating any type of employment relationship or any type of compensation being due to me. 28. I am aware of and agree with the certification policy that requires taking and passing the module I test (HCMBOK® Practitioner) before taking the module II test (HCMP® Expert Professional), even if both training modules have been completed. Completion of module II does not automatically entitle participants to take the exam for the HCMP® Expert Professional certification, which can only be taken after the HCMBOK® Practitioner certification (module I) is obtained.
  29. I am aware that I can only attend Module II after taking Module I of the HCMBOK Training and Certification Program.
  30. I agree that any eventual litigation somehow related to my HUCMI application for certification will be resolved solely in accordance with HUCMI’s rules and procedures.
  31. I agree that when I sign up for certification level HCMBOK® Master Professional will have to submit a case of change management using the guide HCMBOK, according to the standards established by HUCMI, within 90 days after my application for this certification. This case will be evaluated by the certification committee of HUCMI and may be approved automatically or be subjected to audit and to be presented for me by video or audio conference. The HUCMI reserves the right not to approve cases in its sole discretion. In such situations the postulant to the title HCMBOK® Master Professional may submit new case of change management using the guide HCMBOK without any additional cost within 90 days.
  32. I agree that if I am not approved after three attempts at HCMBOK® Master Professional certification, I will have no right to hold a fourth attempt, that is, sending a fourth change management case will not be allowed, unless upon payment of a new registration fee.
  33. I agree that if I am not approved after three attempts at HCMBOK® Master Professional certification, or if I fail to send the change management case within 90 days after my application for this certification I will not be entitled to any refund.
  34. I am aware that the HUCMI has the right to grant or not the HCMBOK® Master Professional certification according to their criteria and that I shall not be entitled to question these criteria in any way.

  36. HCMBOK® Master Professional-certified professionals undertake to register in the HUCMI website a change management case using the HCMBOK guide, following the case description standard defined by HUCMI, every two years. Failure to do so will result in cancellation of the HCMBOK® Master Professional title.
  37. Each case registered by HCMBOK® Master Professional automatically implies payment of a 2-year certificate maintenance fee. Non-payment of this maintenance fee will result in automatic suspension of the HCMBOK® Master Professional certificate and title until the situation is rectified.
  38. I take full responsibility for all information included in the cases registered by me in the HUCMI website free and unencumbered of any charges arising thereof.
  39. HUCMI reserves the right to suspend or revoke the certification granted to any individual who has violated this agreement or committed any act contrary to HUCMI’s Code of Ethics and Professional Conduct. HUCMI follows the Code of Ethics and Professional Conduct published on the ACMP (Association of Change Management Professionals) website, which I declare to know, understand and fully agree with, even in English, published at: https://cdn.ymaws. com/www.acmpglobal.org/resource/resmgr/Website_2015/ACMP_Code_of_Ethics_2014.pdf
  40. I will be solely and fully responsible for my performance as change manager or any related activities, maintaining HUCMI free and unencumbered of any charges arising thereof.
  41. I agree to maintain HUCMI, its directors, administrators, members, examiners, employees, lawyers, representatives, training centers and agents free and unencumbered of, and indemnify them for, any legal actions, law suits, obligations, damages, complaints or claims resulting from or in connection with this application for certification, exam scores or any other action taken by HUCMI, including without limitation, all actions related to ethics-related matters and cases. I understand and agree that any decisions on my qualification for any certification as well as all the decisions on my continued qualification for any certification, and compliance with the HUCMI Code of Ethics and Professional Conduct are at HUCMI’s sole discretion and that these decisions are final.
  42. I understand and agree that this agreement may be changed at any time without notice and also hereby agree with the new agreement to be published on the HUCMI site.
  43. I am aware and agree that the HCMBOK to AGILE - Change Management for Agile Projects training follows all the rules stipulated for the HCMBOK Training and Certification Program, including in relation to the exams, except in this case the training is formulated in a single module and consequently there is a single certification exam.
  44. In the case of courses conducted with live virtual models, I declare that I am aware that the training may be recorded and that my image or audio may be made available along with this video on internet channels and used if HUCMI wishes to. I give up any image or author right that is related to my participation in the published media.
  45. I authorize HUCMI to use my image and audio for virtual courses that will eventually be recorded and disseminated on social networks or other channels via the internet.
  46. All terms stipulated in this agreement are related to the time zone (UTC / GMT -3 hours).
  47. I understand and agree that the teaching material used in any training offered by HUCMI constitutes its intellectual property and will not be shared by the instructor with students.
  48. I agree that the recording of the class by students is strictly prohibited and constitutes a violation of copyright, intellectual property, trademark and/or image rights.
  49. I am aware and agree that training conducted through live virtual means meets the same dynamics as face-to-face training. Classes are not necessarily recorded. Allowing a student to watch a class or part of it through previously recorded videos is a mere liberality of HUCMI. The videos of classes that exceptionally will be shared with students, will only present the part of the concepts presented by the instructor and will be available to students for a maximum period of 24 hours after being shared. In exceptional cases, when due to force majeure the student is unable to participate in a class or part of it, he/she must request access to a recording of the conceptual part presented by the instructor within 24 hours after the date/time of the end of the missed class. It will be up to HUCMI to decide whether the content of a class missed by the student will be shared or not, exclusively according to its evaluation criteria in each case, and the student will not be able for appealing this decision. No student is allowed to request more than 2 classes missed in each HUCMI training.
  50. I agree that in the case of training carried out by virtual means, the teaching material sent to the student will have no additional cost, as long as the delivery address is in the same country that is promoting the training. If the shipping address of the teaching material is from another country, the postage costs will be charged separately.
  51. In the event that the student desists from participating in the training, the paid values will be reimbursed in accordance with the following rules:
    • Cancellation formally communicated to the Training Center by e-mail with up to 15 days in advance. Reimbursement of 100% of the value actually paid, deducted by the receiving costs.
    • Cancellation formally communicated to the Training Center by e-mail with up to 5 days in advance. Reimbursement of 50% of the value actually paid, deducted by the receiving costs.
    • The cancellation formally communicated to the Training Center by e-mail with less than five days in advance of the training will not be reimbursed. In this case, the student may use his credit for training at a future date of any modality – LIVE e-learning or in person for a maximum period of one year.
